HIV 4th Generation Antigen/Antibody Test in Nepal

According to Dr. Kamal Raj Dhital, dermatologist and venereologist with more than 10 years of clinical experience as a dermatologist and in STD doctor in Nepal, HIV testing should be selected according to the type of exposure and the time elapsed since exposure. Radiant STD Clinic in Kathmandu provides HIV rapid testing, HIV 4th-generation testing, and HIV NAAT testing.

What is an HIV 4th-generation test?

The 4th-generation HIV antigen/antibody test detects two markers:

  • HIV p24 antigen – appears relatively early after infection.
  • HIV-1/HIV-2 antibodies – develop as the immune response occurs.

Because it looks for both antigen and antibodies, it can detect HIV earlier than an antibody-only test.

When should you do it?

For a laboratory-based 4th-generation antigen/antibody test using blood from a vein, CDC states that it can usually detect HIV infection 18–45 days after exposure.

A practical approach is:

Time after exposure 4th-gen lab test
<18 days Too early to rely on
18–29 days Can detect some infections, but not all
30 days Very useful, but follow-up may be advised
45 days Generally considered the end of the CDC laboratory window
>45 days A negative laboratory 4th-gen result is highly reassuring; interpretation depends on the clinical situation

A NAT/HIV RNA test detects infection earlier, with a typical window of 10–33 days.

What does a negative result mean?

If your 4th-generation laboratory test is non-reactive/negative, it means HIV was not detected by that test.

However, the timing matters. A negative result obtained before the test’s window period has ended cannot necessarily exclude a very recent infection. CDC recommends repeat testing after the relevant window period when the initial test was performed too early.

What if the result is reactive?

A reactive 4th-generation test does NOT by itself establish an HIV diagnosis. It requires confirmatory testing according to the HIV diagnostic algorithm, generally including an HIV-1/HIV-2 differentiation assay and, when indicated, HIV RNA testing.
